How to Enroll

1. Review the Plan Details.
2. Make sure you are enrolled in Medicare Parts A and B.
3. Download and complete application and payment forms.
4. Contact USC Health Plans to review the application. (Application for enrollment must be received with all necessary documentation no later than the 10th of the month for your effective date to begin on the 1st of next month. If you submit later than the 10th of the month, your effective date will begin on the 1st of the second consecutive month. The effective date always begins on 1st of the month.)
   • Example: If you submit your application on March 5th, your effective date for Senior Care starts April 1st.
   • Example: If you submit your application on March 20th, your effective date for Senior Care starts May 1st.
5. Completed application and required documents must be emailed to healthplans@usc.edu.
